Employment Application

Case Managers provide linkage and coordination services to adult Medicaid recipients with severe disabling mental illness. The purpose of these services is to assist eligible individuals to gain and maintain access to needed medical, social, educational, mental health and other services. They are directly supervised by a program manager.

Individual Qualifications

Case Managers are required to pass a criminal history background check, have no more than 2 driving violations within the past 3 years, and have at least a bachelor degree in a human service field from a nationally accredited university or college . If a person possesses a social work or other licensable degree in the State of Idaho, they must obtain a license prior to being able to work. Unlicensed persons working as a Case Manager must obtain a Certified Psychiatric Rehabilitation Practitioner (CPRP) certificate within 30 months of any experience (including from other agencies.)
