Substance Use Disorder Treatment

We provide outpatient and intensive outpatient services for individuals struggling with substance use disorders (SUD).

According to SAMHSA, Substance use disorders occur when the recurrent use of alcohol and/or drugs causes clinically significant impairment, including health problems, disability, and failure to meet primary responsibilities at work, school, or home.

DUI Evaluations

DUI evaluation to take approximately one hour. In many cases, a completed report can be provided the same day once all necessary documentation has been received. The BAC and/or lab results needs to be provided at the time of the appointment. Services are provided in person at our Boise office. The total fee is $150. To secure your appointment, it requires a $50 non-refundable deposit, the remaining balance of $100 is due at the time of service.

How to get started

  • To get started, please contact our reception team at (208) 321-0160. Our staff will guide you through a brief intake process, which includes gathering basic information about the individual seeking services, the type of support they are requesting, and relevant insurance details (if applicable). We will also discuss scheduling preferences and any additional information you wish to share to help us best support your treatment needs.

    When sending a referral, please fax the referral to (208) 321-0221. 

    Make sure to include the information above.

  • Once our receptionist collects the necessary information to begin the Substance Use Disorder (SUD) program, you will receive an intake packet either by email or in the mail. 

    This packet must be completed prior to your first appointment. If you are unable to complete it beforehand, please arrive 30 minutes early to finalize the paperwork before your intake assessment.

     

    If your treatment is court-ordered, please ensure you bring all required documentation with you. 

    A signed release of information will also be needed for any individuals or agencies you wish to involve in your treatment.

  • After completing all required paperwork, you will attend your assessment appointment. 

    This may include a Comprehensive Diagnostic Assessment, ASAM, or DUI Evaluation. These appointments typically last between one to two hours. 

    Please plan to be present in person for the entire duration of your assessment.

    At the conclusion of your appointment, you will check out with our receptionist, who will assist you in scheduling your follow-up appointments as recommended by your assessor.

    Once this process is complete, you are officially set to begin services. 

    Our SUD Administrative Team will contact you only if additional information is needed; otherwise, you will continue with your scheduled appointments as outlined during your initial assessment.
